2012 Fees and Registration forms now available

Hope you have enjoyed your time off of track!! It is almost that time again for the 2012 track season. Please keep in mind we are doing payment plans for anyone interested in paying in advance. See notes on fee sheet.
Registration Forms:
Even if you completed the forms last year you will have to complete pages 3 and 4. (parent permission form and release form)
If any changes to your child(s) medical history please complete page 2. (Medical History form, parent permission form, and release form)
If any changes to your demographic information please complete page 1. (registration form, parent permission form, and release form)You can either email the revised or updated registration form back to me or on the first day of practice (date coming soon) bring forms with you. Keep in mind I made the form a type-able form but you do have to sign the form before you return it to me.
***If you have anyone new coming out this year. Please pass on the information.
